Send the complete name, address, and phone number of the band, as well as the name of the person chosen to be spokesperson or leader of the group.

Send a biography of the band or individual that lists any and all activity and history that would show experience you may have as well as any connections you have made.

Your CD must contain a minimum of four songs, and your best song should be first on the CD. A photo of the band or individual is optional, but would be appreciated.
